CCO Case Study Submission

Please fill out this form to request a case to be reviewed by CCO and presented during our “Case Studies with CCO” webinar. If you are a CCO Club member you can attend at no cost. If you are not you may catch it on simulcast via Facebook or YouTube. In addition we will notify you once complete – regardless of club status.

• Case Studies with CCO  – this is where we take your cases and review your coding. See how an experienced coder takes a chart and analyzes it and codes it.

Please make sure your copy and paste or PDF attachment is free from any personal health information (PHI) – white out is best. To be clear – we should not see a patient name, identifier (chart ID), physician name or facility name. Hand written cases are acceptable as long as they are fully redacted.
